How Could You? Hall of Shame-Wales-Seren Bernard case-Child Death

From Pembrokeshire, Wales, “14-year-old [foster child] Seren Bernard took her own life in April 2012.

“A mother whose teenage daughter was found dead whilst in the care of Pembrokeshire social services says she still doesn’t have all the answers.”

“Sarah Pollock has continued to campaign for access to the full report by the council into her daughter’s death, which she says she’s been denied.”

“No parents should have to fight to find out what went wrong with their child and this document is being refused on grounds of confidentiality and sensitivity. Well surely I’m entitled to know everything about my daughter?

So far she’s only been allowed to see a summary and two years ago she also called for an independent review into the teenagers death.

Seren began to have problems during her teenage years, she went missing whilst living with relatives and when she returned she was placed in the care of foster parents.

An inquest at the time ruled that failings by Pembrokeshire social services didn’t contribute to her death.

Pembrokeshire County Council told ITV Cymru Wales it is following Welsh Government guidelines by not making the full report public.

The death of a child is a profound lost and all the professionals involved in the tragedy feel great sympathy for Seren and her family but the authority is following Welsh Government guidelines by not making public the full overview report into the death of Seren Bernard.
